Intralase is the safest method for creating the corneal flap. Using the IntraLase FS laser to create the corneal flap with a safe laser beam instead of a blade.
Patient Surgery Instructions
PRE-PROCEDURE Please remember the following:
Consent form Read over the packet that will be given to you, and initial each page of the
consent form prior to arriving for your procedure. Please leave the last page blank
until you arrive so we can witness your signature.(Consent form must be complete & signed prior to
procedure)
Prescriptions Please remember to bring your prescriptions (antibiotic eye
drops and mild sedative) with you on your procedure date. You will not start using drops until after
your procedure. Without a prescription plan, expect to pay $40-60 for the antibiotic drops.
Transportation Please make arrangements for a driver for your procedure date and
in some instances 1-day checkup.
Arrival time Please arrive 5-10 minutes early for your appointment. Expect to be
in the office for at least two hours, and ensure your ride is flexible if there are delays. Surgery will begin
1-1.5 hours after your arrival, so a book, music or activity is suggested for waiting periods during this time.
Contact lenses Please remember to discontinue contact lens wear, as detailed below:
Soft or toric lenses out 7 days
Gas permeable lenses out 4 weeks for up to 10 years of wear plus 1 week for each additional 10 years of wear
PMMA (true hard) out 12 weeks
Restrictions Please refrain from wearing any alcohol-based products such as hairspray, mousse,
cologne/perfume, or moisturizers - as they can affect the laser. NO MAKEUP on the day of your procedure and remove all
eye makeup extremely well the night before.
Payment Full payment (or financing contract) is required on the day of the booking of the procedure.
We accept money orders, cashiers checks, VISA and Mastercard. NO PERSONAL CHECKS accepted. We do not
accept Cash; a cashiers check or certified check is required in lieu of Cash.
Financing Please ensure that your Care Credit or other financing contract(s)
has been received by our office prior to date of surgery.
POST-OPERATIVE APPOINTMENTS:
Routine post-operative visits are scheduled at 1 day, 1-2 week, and 1-3 months following surgery.
It is vital that the 1-day post-op appointment occur at the original surgery center.
We will be happy to refer you to one of our co-managing optometrists if you prefer to have your extended
post-operative care performed in your area.
POST-OPERATIVE NOTES:
Plan on at least 1 day of recovery for Lasik, and up to 5-7 days for PRK.
Refrain from rubbing, scratching, or pressing on the eyes.
Avoid reading, computer work and other activities that might involve undue eyestrain.
Refrain from wearing all makeup for 1 week, and eye makeup for 2 weeks (due to rubbing).
Refrain from swimming in pools/hot tubs for two weeks, and be cautious about water in the eyes (shower) for 1 week.

**CANCELLATION POLICY: As we often run a Waiting List on surgery days, if you must reschedule your appointment,
a 48-hour notice is required!
